I wear a large (about a 42). I read in the reviews that one person that wears a large said he should have gone with an extra large. Any comments?

Most reviews seem to indicate the thing being bigger than a normal shirt size. i'm 3xl shirt, 325 lbs; should i get 2xl or 3xl?

I would get the 3x. If it turns out slightly large the strap will pull it in. Their was a size chart toward the bottom of the listing. Check that.
